この本は新版が出ています。(感染症は実在しない (インターナショナル新書))
ご本人が「むしろ2020年のコロナウイルス問題にこそ、本書のような考え方が必要なのです」と言っているので読んでみました。正直ホッとしました。妻が子宮がん検診に行くと言って気まずい雰囲気になっていたのです。
病気は「モノ」ではなく「コト」である。ウィルスが「ある(いる)」こと、血圧が高いことと病気とは直接の関係はない。病気とは人間が恣意的に規定した決まり事(現象)だから、「科学的に正しい病気とか正しくない病気とかいう「事実」「実在物」ではないのです。(中略)検診を受けなければならない、あるいは受けてはならないとか、こういう治療をしなければならない、治療をしてはいけないといった制約や決めつけが根拠を失ってしまう」(P.140-141)のです。
私は、近藤誠さんの本に衝撃を受け、「がんを治療するのは苦しいだけで無意味だ」(だから)「がん検診も害だけがあって無益だ」と思っています。でも、他人にその「考え」を押し付けることには自分でも納得できない部分がありました。私自身も、もし「がんです。余命あと3ヶ月です。」と言われたらきっと動揺するでしょう。だから検診は受けません(笑)。
医療は自分の価値観(健康とか安心とか長寿とか)と医療(による利益・不利益)との価値交換です。価値観は人ごとに違いますから、治療や検査を受けるのかどうかも人によって違っていいのです。大事なことは、検査・治療・投薬の利益・不利益の情報を的確に医療者は提供し、患者(?)は理解することです。
たしかに今の日本では決定権は患者にあります。でも、患者と医者は対等な関係ではないですよね。情報を握っているのは医療者です。英語はたしかに知っているに越したことはありません。外国語は私の劣等感の大きな部分です。翻訳文化の日本では大概のものは日本語訳が出ています。だけど、翻訳されるのは「売れる」「メジャーな」本だけです(それでも読みきれるものではない)。そして、翻訳は翻訳者の、あるいは現在の学術の成果というバイオスがかかります。「新訳」はきりがないのです。その上翻訳には時間がかかりますし、高い(!)。患者は日本の医療雑誌を読むこともできないし、英語だって読めないのです。それなのに「自分で決めなさい」というのはある意味無責任です。その後訴訟を起こしたって、勝てる可能性は殆どありません。裁判そのものが「権威」の上に成り立っているのです。それなら、近藤さんのように「検査・治療を受けるな」と言い切ったほうが親切かもしれないのです(それで早死しても、どうせ早死かどうかは「科学的に」証明することができないのだから(笑))。
医療も(生命)保険と同じで、人々の不安に付込む商売です。だから「健康保険」という制度が可能なのですよね。大切なことは病気になっても、働けなくなっても、死んでも自分や家族が困らないという「セーフティネット」なのではないでしょうか。医療者が間違っても、患者が間違ってもいい(もちろん、最善を尽くすという前提ですが。)ということが必要だと思います。また、検査を受けるお金がない、検査を受けて病気だと言われたら家族が路頭に迷う、ということもあると思います。それでは、決定権は無意味ですよね。
「病気だ」「病気じゃない」というのが0か1かじゃないということは、それ以外のことについても言えると思います。少なくともそれを決定するのは医療者でもないし、もちろん行政でもありません。基準を決めることで、医者も行政も「無責任」で居られます。現代の「免罪符」です。。高血圧症の基準を150から130に下げたことで、突然数千万人が「病気」になりました。なぜ、そんな事をしたのでしょう。国民のためを思って?その可能性は低いと思います。行政はそれで得をするわけでもありません。むしろ、費用がかさんで大変になったと思います。それでも基準を下げたのは、それで儲かる人がいるからだと思うのが自然だと思います。儲かったのが誰かは考えればわかりますよね。血圧は高くても低くても人体に影響があります。でも、それは人体の自然な反応であることを認識する必要があります(風邪などで体温が上がるのは体の自然な反応です。体が赤信号を発しているとも言えるし、治そうとしているとも言えます。それを解熱剤で下げることが体にとっていいことかどうかは単純に言えることではありません)。歳を取れば当然血圧は高くなる傾向がありますよね(体は無意味に熱を出したり血圧を高くしているわけではありません)。だから、血圧が高いということでどのようなリスクがいくら上がるのか、ということと、薬を飲むのこととの価値の比較が必要なのです。
「価値」というのは経済学的な意味ではありません。ですが、貨幣経済がすべてを覆い尽くしている社会では、すべての判断は経済的価値の様相を帯びます。)
まだまだこの本から汲み取るべきものはあると思います。納得できない、理解できないこともいっぱいです。今、新型コロナウィルスがマスコミも個々人の心も覆っています。
「どの方法をとっても新型インフルエンザのリスクをゼロにすることは不可能です。ゼロリスクは、求めてはいけない非現実的な目標です。また、それを求めると失うものが多くなりすぎるのです。
だから、マスクを適切に着用してインフルエンザのリスクを回避するのは、これはまっとうなやり方でしょう。でも、マスクが買い占められたがゆえに買えなくなって絶望的な気分になったり、マスクをしていないからといって白い目で見られたり糾弾されたり、これは本来の目的から照らし合わせるとむしろマイナス面が多くなってしまう、本末転倒な状態でしょう。健康のためにマスクをしていたのに、このようなストレス下では不健康になってしまいそうです。」(P.202)
今のコロナの話ではありません。11年前の本です。多分著者の考えは当時と変わっていまいと思うのですが、「まえがき」を読むためだけでも新版を買う「価値」があるな、と思っています。

However, only "selling" and "major" books are translated (still not readable). And translation takes the bios of translators or current academic achievements. The "new translation" is endless. Besides, translation is time consuming and expensive (!). Patients can't read Japanese medical magazines, nor can they read English. However, it is irresponsible in a sense to "decide for yourself". If you file a proceeding after that, there is little chance of winning. The trial itself is built on "authority." In that case, it may be kinder to say "Don't undergo examination / treatment" like Mr. Kondo (so even if you die prematurely, you cannot "scientifically" prove whether you die prematurely (" Lol)).
Medical care, like (life) insurance, is a business that takes advantage of people's anxieties. That's why a system called "health insurance" is possible. The important thing is a "safety net" that will not bother you or your family even if you get sick, can't work, or die. I think it is necessary that the medical staff is wrong and the patient is wrong (assuming that we will do our best, of course). "It is impossible to eliminate the risk of pandemic flu by any method. Zero risk is an unrealistic goal that should not be sought, and if you do, you will lose too much.

However, I feel desperate because I can't buy the mask because it's bought up, or I'm seen with white eyes or denounced because I don't wear the mask.
